You have the right to receive complete, accurate information about your diagnosis, treatment options, potential risks and benefits, and expected outcomes — in language you can understand.

Right to Participate in Your Care

You have the right to participate in decisions about your treatment plan, including the right to accept or refuse any recommended treatment, and to ask questions at any time.

Right to Privacy and Confidentiality

You have the right to have your health information kept confidential and used only for your treatment, payment, and healthcare operations, as described in our Notice of Privacy Practices.

Right to Access Your Records

You have the right to access, review, and request copies of your medical records. You may also request amendments to your records if you believe information is inaccurate or incomplete.

Right to File a Complaint

You have the right to voice concerns or file a complaint about your care without fear of retaliation. We take all complaints seriously and will respond promptly and fairly.

Right to Non-Discrimination

You have the right to receive care without discrimination based on race, color, national origin, sex, age, disability, religion, sexual orientation, or any other protected characteristic.

Right to a Second Opinion

You have the right to seek a second opinion from another qualified healthcare provider at any time. We support your right to make fully informed decisions about your care.

Patient Responsibilities

To help us provide you with the best possible care, we ask that you:

  • · Follow the treatment plan prescribed by your physician and communicate any concerns promptly.
  • · Notify us if you experience any adverse reactions or changes in your health status.
  • · Treat all healthcare providers and staff with courtesy and respect.
  • · Fulfill your financial obligations for services received.
